Practitioners and pharmacists must perform the following steps to request an account:
You may submit your access request form at this time. If your access request is approved, you will receive two e-mail notifications that will include instructions for accessing the database. If your access request is not approved, you will be notified by the WA Prescription Review program staff.
Once you’ve received your instructions, please click on the Practitioner & Pharmacist Query Site link NOT the Access Request Site link to access the database.
